Participants will receive a flash drive that will contain the agenda, program, participant list, and handouts. The agenda and program will also be available in print. If you need an alternate format (requests for braille and large print must be submitted by 2/19/14), please select one preference below:

AFB Leadership Conference Registration Fees

 
Conference Fees 
One day: $150 (After February 3 $175) 
Two days: $250 (After February 3 $300) 
Three days: $300 (After February 3 $350)

AFB, in conjunction with Audio Description Associates, announces AUDIO DESCRIPTION TRAINING � February 26-28. This special training program will run concurrently with the Leadership Conference. Because this program runs from Wednesday through Friday, you may choose to attend just the Audio Description Training or the Audio Description Training and the Leadership Conference on Saturday. Please choose one of the two options below. 
 
*Three Day Audio Description Training: $395 
*Three Day Audio Description Training plus Saturday AFBLC & Access Awards: $545 
 

Access Awards and Migel Awards

No charge for these events.  
The Access Awards will take place the afternoon of Friday, February 28.  

Your registration fee includes the following meal functions:
 
  • Access Awards reception on Friday evening (consisting of light hors d'oeuvres)
  • Migel Awards luncheon on Saturday
 

Payment Through Purchase Order or Check

 
Checks: Please make checks payable to the American Foundation for the Blind (AFB Leadership Conference). 

Purchase Orders: State agencies may use a purchase order if necessary. 

If you have chosen to pay by check or purchase order, please print out this page and mail it in with your payment. We will send a confirming email once the payment process is complete.

You may combine multiple participants onto one check or purchase order but please print out a separate registration form for each person. We will send an invoice for those sending purchase orders.

After your registration is complete, you will receive an e-mail from AFB, which will serve as your receipt.
