If you submit your evaluation to AFB via e-mail, your name will be removed from the e-mail before the responses are tallied to ensure confidentiality.

Note: If you wish to apply for CEUs from the sessions you attended, please complete the CEU request form you will find after submitting your evaluation form. The evaluation form is a required element of the CEU documentation.
